1. To be treated with respect, dignity, and consideration including the recognition of personal beliefs and values.

6. To privacy and confidentiality, and to receive treatment in private as well as care for personal needs.
7. To coordination and continuity of your healthcare.
8. To know the identity of providers, nurses and others involved in your care.
9. To have information presented in terms you can understand, including treatment and care options.
10. To participate or have your representative participate in the development of and decisions regarding your health care plan, and to be offered information on making a health care or advanced directive.
11. To reasonable access to healthcare.
12. To access health care records according to the health center’s policy and procedure.
13. To be informed of the patient complaint process and be heard if problems, complaints, or grievances arise.
14. To be informed of charges for services as well as payment options.
15. To change your provider if other qualified providers are available.
16. To not be subjected to abuse, neglect, exploitation, coercion, manipulation, sexual abuse, sexual assault, or restraint or seclusion .
17. To not be subjected to retaliation for submitting a complaint to Arizona Department of Health Services.
18. To not be subjected to misappropriation of personal and private property by personnel member, employee, volunteer, or student
19. That except in an emergency, you, or your representative must consent to or refuse treatment; you or your representative may refuse or withdraw consent for the treatment before the treatment is started.
20. Except in an emergency, to be informed of alternatives to a proposed psychotropic medication or surgical procedure including possible risks and complications.
21. To give consent to photographs before your picture is taken.
22. To receive a referral to another health care provider if we are unable to provide physical or behavioral health services that you need.
23. To participate or refuse to participate in research or experimental treatment.

1. To provide complete and accurate medical information to your health care providers and report any changes in your health.
2. To ask questions so that you have a clear understanding.
3. To make informed decisions.
4. To understand your health problems and follow agreed upon plans and instructions for your care.
5. To recognize the impact of your lifestyle choices on your personal health.
6. To be responsible for your actions if you do not follow the provider’s instructions.
7. To keep scheduled appointments, reschedule in a timely manner or cancel your appointment within 24 hours of appointment time.
8. To respect the rights, privacy and confidentiality of other patients and clinic personnel.
9. To accept financial obligations and understand your health insurance benefits.
10. To inform your provider about any living will, medical power of attorney or other directive that could affect your care.
11. To be photographed one time and have that photo placed in your chart for identification purposes OR to be prepared to show photo identification at each visit for patients 18 years or older.
12. To be considerate of the rights of other patients and personnel.
13. To follow the rules of required patient and visitor conduct. These requirements ban the use of threatening, offensive, or abusive language, or activities.
14. The use of tobacco, alcohol, or illicit substances are also banned while on Desert Senita property.
